Who We Are
Mercy Home Health is a member of Trinity Health At Home, a national home care, hospice and palliative care organization serving communities throughout eleven states. As a faith-based, not-for-profit organization, we serve patients and families in the comfort of home, offering various home health care services. Mercy Home Health is licensed, Medicare certified and accredited by the Joint Commission.
Our Services
Home Care
We provide care to individuals recovering from illness or surgery and help manage chronic conditions. We are committed to enhancing the overall well-being of our patients. With our own virtual care program, patients benefit from 24/7 monitoring in real-time and are empowered to achieve health care goals and avoid preventable hospitalizations.
Palliative Care
We relieve suffering and improve the quality of life for individuals facing serious illnesses. We work closely with patients, families and health care professionals to develop personalized care plans that address physical, emotional and spiritual needs.

Our History
Mercy Home Health's legacy began in 1910 when two dedicated Visiting Nurse Associations joined forces to serve the underserved in Delaware County, Pennsylvania. Nearly century later, in 1994, we evolved into Mercy Home Health. Today, we proudly extend our compassionate care across the entire Greater Philadelphia Area, continuing our mission to support and uplift our community with the same dedication and commitment that inspired our founding.
