Who We Are
St. Mary Home Care & Hospice is a member of Trinity Health At Home, a national home care, hospice and palliative care organization serving communities throughout eleven states. As a faith-based, not-for-profit organization, we serve patients and families in the comfort of home, offering various home health, palliative care and hospice care services. We are licensed, Medicare certified and accredited by the Joint Commission.
Our Services
Home Care
We provide care to individuals recovering from illness or surgery and help manage chronic conditions. We are committed to enhancing the overall well-being of our patients. With our own virtual care program, St. Mary Home Care & Hospice patients benefit from 24/7 monitoring in real-time and are empowered to achieve health care goals and avoid preventable hospitalizations.
Palliative Care
We relieve suffering and improve the quality of life for individuals facing serious illnesses. We work closely with patients, families and health care professionals to develop personalized care plans that address physical, emotional and spiritual needs.
Hospice Care
We provide specialized end-of-life care and support to individuals with terminal illnesses, focusing on pain management, symptom relief, emotional comfort and spiritual guidance. Our interdisciplinary team of experts are committed to honoring the dignity and wishes of our patients while offering compassionate support to their loved ones.
Bereavement Services
Navigating the profound period of grief after losing a loved one requires understanding, support and community. We offer free bereavement services to anyone in our communities who has lost a loved one. Our specialized grief support groups address various aspects of the grief journey. Experienced bereavement counselors provide personalized sessions and our ongoing adult grief support group fosters a supportive environment. We also host community memorial events to honor and remember loved ones we’ve lost.

Our History
St. Mary Home Care’s legacy began in 1910 when two dedicated Visiting Nurse Associations joined forces to serve the underserved in Delaware County, Pennsylvania. Nearly century later, in 1994, we evolved into St. Mary Home Care. Today, we proudly extend our compassionate care across the entire Greater Philadelphia Area, continuing our mission to support and uplift our community with the same dedication and commitment that inspired our founding. In 2024, we expanded our home health and palliative care services to include hospice.

As we move forward, our focus remains to bring compassionate, high-quality care to patients in the comfort of home. In the coming years, we will expand our services and serve more families who need us. Our goal remains the same: to be your trusted home health and hospice care provider.
