Who We Are
Saint Agnes Home Health and Hospice of California is a member of Trinity Health At Home, a national home care, hospice and palliative care organization serving communities throughout eleven states. As a faith-based, not-for-profit, we serve patients and families in the comfort of home, offering various home health care services.
Our Services
Home Care
We provide care to individuals recovering from illness or surgery and help manage chronic conditions. We are committed to enhancing the overall well-being of our patients. With our own virtual care program, patients benefit from 24/7 monitoring in real-time and are empowered to achieve health care goals and avoid preventable hospitalizations.
Palliative Care
We relieve suffering and improve the quality of life for individuals facing serious illnesses. We work closely with patients, families and health care professionals to develop personalized care plans that address physical, emotional and spiritual needs.
Hospice Care
We provide specialized end-of-life care and support to individuals with terminal illnesses, focusing on pain management, symptom relief, emotional comfort and spiritual guidance. Our interdisciplinary team of experts are committed to honoring the dignity and wishes of our patients while offering compassionate support to their loved ones.
Bereavement Support
Navigating the profound period of grief after losing a loved one requires understanding, support and community. We offer free bereavement support to anyone in our communities who has lost a loved one. Our specialized grief support groups address various aspects of the grief journey. Experienced bereavement counselors provide personalized sessions and our ongoing adult grief support group fosters a supportive environment. We also host community memorial events to honor and remember loved ones we’ve lost.

Our History
Our Beginning
Saint Agnes Home Health and Hospice began when the Sisters of the Holy Cross founded Saint Agnes Medical Center nearly a century ago. Our founders saw a need for home-based care within the Central Valley communities and believed our organization could play a significant role in lives of those who needed our health care services. Since its founding, Saint Agnes Home Health and Hospice has grown to serve thousands of patients and has been a pillar in the Central Valley communities.
Our Key Milestones
1978: In our early years, we expanded our services to include hospice care to support individuals nearing the end of life.
2016: Launched our virtual care services through Trinity Health Home Care Connect™ and began monitoring patients through our secure remote technology. This program still exists today. Learn more.
2018: Saint Agnes Hospice celebrated 40 years of bringing compassionate hospice care to Central Valley families.
2023: Saint Agnes Hospice celebrated 45 years of bringing compassionate hospice care to Central Valley families.
Our Recent Accomplishments
More recently, in 2024, we expanded our services to include palliative care to help individuals with serious illnesses manage pain and symptoms and improve overall quality of life. Learn more.
