Our History
In 1902, visionaries in Indiana planted the seeds of what would become a multi-state network of compassionate care. Determined to bring care and hope directly to people's homes, they founded the Visiting Nurse Association, now known as Saint Joseph Health System VNA Home Care. This marked the beginning of Trinity Health At Home's story.
Across the country, similar visions took root. Caring people saw the need for compassionate, home-based care in their communities and started home health agencies. Over time, these organizations united to form Trinity Health At Home, which now serves eleven states. More than a century after our first agency began, our legacy of service and innovation continues to grow.
Our Roots in Compassion
Every Trinity Health At Home agency has its own story, but they all share a common thread: a desire to help others. Many of our agencies were started by Catholic sisters, such as the Sisters of the Holy Cross, the Sisters of St. Francis and the Felician Sisters. Others were founded by caring community members who saw a need and stepped up to meet it.
Over the years, we've adapted to many changes, from new medical technologies to global health crises. But one thing remains constant: our commitment to compassionate care. The same spirit of kindness and mercy that inspired our founders still guides us today.

Trinity Health Home Care Connect
Since 2016, this virtual care program has offered round-the-clock remote patient monitoring. This allows for real-time interventions and reduces unnecessary hospitalizations.
Beyond Balance
This program helps people who are at risk of falls live safely at home through risk assessments, physical therapy, home safety evaluations and education.
Cardiac Partners
For patients needing cardiac care, this program offers comprehensive services to improve heart health and overall quality of life.
We also offer specialized programs addressing concerns such as diabetes management, incontinence and wound care. By combining advanced technology with our compassionate approach, we make sure our patients receive the best possible care at home.
