Getting Started


MyChart offers you personalized and secure online access to your medical records. It securely enables you to use the internet to help manage and receive information about your health. As a home care, palliative care or hospice patient, you can use MyChart to:

  • View select information in your health record.
  • View upcoming visit dates.
  • View your current medication list and care plan.
  • Review past visit summaries and notes.
  • Access trusted health information resources.

Download the Trinity Health MyChart app to keep track of your health using your smartphone and/or tablet. Note: some features are not available when accessing your account via the app.

Primary care patients of Holy Cross Health and Medical Group will also be able to:

  • Schedule appointments and view future appointments including video visits. 
  • View test results.
  • Request prescription refills.
  • Pay your bills online.
  • Communicate privately and securely with your doctors.
  • Confirm your appointment and electronically check-in to complete pre-visit paperwork online before your appointment.
  • Be added to a waitlist to be notified of earlier appointment openings.
  • Get care through video visits.

No. MyChart is a free service offered to our patients.

We encourage you to sign up for your MyChart account. At your visit, you will receive a MyChart instant activation link, valid for 14 days, via text or email. Another way to register is using an activation code found on your After-Visit Summary sheet or Billing Correspondence. Go to mychart.trinity-health.org and register using the activation code.

You will need a computer or mobile device (IOS or Android) that can connect to the Internet and an up-to-date browser, such as Google Chrome, Microsoft Edge, Safari, or Firefox. Use of Internet Explorer is not recommended.

You must be 12 or older to use MyChart. MyChart users 12-17 years of age must have parental or guardian consent or be an emancipated minor.

If your MyChart account is with a different health care system or provider, you will need to create a new account specific to Holy Cross Health. MyChart is utilized by many health care providers and systems throughout the country. Fortunately, if you have an active patient portal account at another organization that uses MyChart, you will be able to link your accounts.

Using MyChart


MyChart provides a summary of your health information based on your home care or hospice team’s entries in your patient portal. You can view your future and past appointments, current care plan, health issues, medications you are taking, allergies, immunizations, and medical history. Note: some features are only available to primary care patients of Holy Cross Health and Medical Group.

 

Privacy


Your name and email address will be treated with the same care and privacy given your health records and will never be sold or provided to anyone else. MyChart complies fully with federal and state laws related to privacy.

The MyChart Support Line, 1-301-754-7363, is available to answer your questions and assist with access 7 days a week, from 7 a.m. to 7 p.m. Eastern Time (including holidays).
