  • In-home visits from skilled nurses to provide personalized care and support.
  • A collaborative partnership with the patient's physician.
  • In-depth nutritional assessments and guidance to promote healthy eating habits.
  • Customized education on topics like blood sugar monitoring and medication management to help patients and caregivers understand and manage the condition.
  • Round-the-clock remote monitoring and access to virtual nurses through our Trinity Health Home Care Connect™ Virtual Care program for eligible patients.
  • Regular foot assessments and education to lower the risk of foot-related diabetes complications.
