Who We Are
We are a national home care, hospice and palliative care organization serving communities throughout eleven states. As a faith-based, not-for-profit organization, we serve patients and families in the comfort of home, offering various home health, palliative care and hospice care services. We are licensed, Medicare certified and accredited by the Joint Commission.
Our Services
Home Care
We provide care to individuals recovering from illness or surgery and help manage chronic conditions. We are committed to enhancing the overall well-being of our patients. With our own virtual care program, Trinity Health At Home patients benefit from 24/7 monitoring in real-time and are empowered to achieve health care goals and avoid preventable hospitalizations.
Palliative Care
We relieve suffering and improve the quality of life for individuals facing serious illnesses. We work closely with patients, families and health care professionals to develop personalized care plans that address physical, emotional and spiritual needs.
Hospice Care
We provide specialized end-of-life care and support to individuals with terminal illnesses, focusing on pain management, symptom relief, emotional comfort and spiritual guidance. Our interdisciplinary team of experts are committed to honoring the dignity and wishes of our patients while offering compassionate support to their loved ones.
Bereavement Support
Navigating the profound period of grief after losing a loved one requires understanding, support and community. We offer free bereavement support to anyone in our communities who has lost a loved one. Our specialized grief support groups address various aspects of the grief journey. Experienced bereavement counselors provide personalized sessions and our ongoing adult grief support group fosters a supportive environment. We also host community memorial events to honor and remember loved ones we’ve lost.

Our Beginning
Trinity Health At Home Southeast Michigan’s (previously known as Saint Joseph Mercy Home Care and Hospice) legacy began when The Sisters of Mercy and the Felician Sisters founded the Trinity Health Michigan regional health system more than a century ago. Our founders saw a need for home-based care within the Southeast Michigan communities and believed our organization could play a significant role in the lives of those who needed our health care services. Since its founding, Trinity Health At Home Southeast Michigan has grown to serve thousands of patients and has been a pillar of the Southeast Michigan communities.

Founded in 1982, Trinity Health At Home Grand Haven (formerly Hospice Care of North Ottawa Community) began as a standalone hospice care agency. It later became a member of Trinity Health Grand Haven. We were the first hospice provider in the lakeshore region. Our founders were non-clinical community members who recognized the need for compassionate end-of-life care within the lakeshore community. Our founders believed our organization could play a significant role in the lives of those who needed our health care services. Since its founding, Trinity Health Grand Haven has grown to serve thousands of patients and has become a pillar of the Tri-City community.

2023: After 41 years, we joined Trinity Health and became a member of Trinity Health At Home.
