Before You Begin


Here are three simple steps to get started with Trinity Health At Home:

Workday


Workday is our human resources application for all colleagues. Workday will be your go-to location for important information.

  • Check your email for your offer letter and review and sign it.
  • Complete additional tasks such as entering your date of birth, marital status, and Social Security number.
  • You'll receive separate emails with your Workday username and password. Use them to log into Workday.

Complete your I-9 Form


This step can be completed virtually through our partner, Equifax, and should be completed prior to your start date. Review the Virtual I-9 Process Guide for assistance in utilizing the virtual verification method.

  1. Go to the I-9 Form site:

  2. Select Trinity Health At Home as your location in the dropdown box.
  3. Enter Section 1 Information
    • Include all last names: You should include all last names you have used, including your maiden name (if applicable).
    • Include hyphens and apostrophes: If your name has a hyphen or apostrophe, you will need to include it.
  • Virtual Section 2 Verification
    • Your I-9 Section 2 Verification will be done virtually using video conferencing/virtual meeting technology. After completing Section 1 of the I-9, you will upload a photo of the front and back of your documentation, then you will click to meet with a virtual agent to verify your documents.
    • The hours to meet with a virtual agent are M-F 5am-4pm PST/8am-7pm EST.
      • If you are completing Section 1 outside of these hours you will be notified to return to complete the portion of meeting with a virtual agent during the hours of operation.
    • To be prepared for the virtual I-9 Section 2 Verification, be in a quiet well-lit room as the agent will be confirming your identity on camera. Have your documents ready to share with the agent and ensure your camera and microphone are turned on.
  • From beginning Section 1 of the I-9 to finishing Section 2 with a virtual agent, it should only take between 10-15 minutes. Please set this time aside so you can finish the process in full.
  • Please visit the Official Website of Homeland Security for a list of acceptable documents.

As a new employee, you must complete the following health requirements:


  1. Drug Testing: You are required to pass a mandatory drug test
  2. Vaccinations and Tests:
    • MMR (Measles, Mumps, Rubella) vaccination
    • Tetanus/Tdap vaccination
    • Varicella (Chickenpox) vaccination, if applicable
    • TB Test
    • Hep B series/declination (if applicable)
    • Flu Vaccination (if required by State)

If you do not have your vaccination records, titers will be drawn to check your immunity.

 

Background Check Requirements


  1. Federal and Criminal Records Check
  2. Sex Offender Registry
  3. Fraud and Abuse Control Information System
  4. Nationwide Criminal Check
  5. Healthcare Provider Check (Nurse Aide Abuse Registry) for DE, IA, IL, MI, OH, PA.
  6. Fingerprinting required for CT, GA, MI, OH and PA (FBI fingerprinting for non-PA residents or PA residents that have lived in state less than 2 years)
  7. (Pennsylvania Only) PA Child Abuse Clearance

 

Additional Verifications


  1. Credential/Certification Verification
  2. Education Verification
  3. Previous Employment Verification
  4. CPR Certification (All clinicians in the states of CA, DE, GA, IL, IN, MD, OH, PA must hold valid CPR certification)

To ensure the health and safety of colleagues and patients you may be asked to provide the following documentation:


  • Most up to date COVID Vaccination – with the exception of a signed declination
  • Flu Vaccine – Flu season with exception if you have an approved exemption
  • TB Testing
