Who We Are
St. Mary’s Home Health & Hospice of Georgia is a member of Trinity Health At Home, a national home care, hospice and palliative care organization serving communities throughout eleven states. As a faith-based, not-for-profit, we serve patients and families in the comfort of home, offering various home health care services.
Our Services
Home Care
We provide care to individuals recovering from illness or surgery and help manage chronic conditions. We are committed to enhancing the overall well-being of our patients. With our own virtual care program, patients benefit from 24/7 monitoring in real-time and are empowered to achieve health care goals and avoid preventable hospitalizations.
Palliative Care
We relieve suffering and improve the quality of life for individuals facing serious illnesses. We work closely with patients, families and health care professionals to develop personalized care plans that address physical, emotional and spiritual needs.
Hospice Care
We provide specialized end-of-life care and support to individuals with terminal illnesses, focusing on pain management, symptom relief, emotional comfort and spiritual guidance. Our interdisciplinary team of experts are committed to honoring the dignity and wishes of our patients while offering compassionate support to their loved ones.
Bereavement Services
Navigating the profound period of grief after losing a loved one requires understanding, support and community. We offer free bereavement services to anyone in our communities who has lost a loved one. Our specialized grief support groups address various aspects of the grief journey. Experienced bereavement counselors provide personalized sessions and our ongoing adult grief support group fosters a supportive environment. We also host community memorial events to honor and remember loved ones we’ve lost.

Our History
Our Beginning
Founded in 1970 as St. Mary’s Home Care, we began as a home-based care program out of St. Mary’s Health System in Athens, Georgia, becoming the second hospital-based home care agency in the state. Our founders saw a need for home-based care within the Athens community and believed our organization could play a significant role in the lives of those who needed our health care services. Since its founding, St. Mary’s Home Health & Hospice has grown to serve thousands of patients and has become a pillar in the Athens community.

Our Key Milestones
2023: After 53 years, we joined Trinity Health and became a member of Trinity Health At Home.
2023: Launched our virtual care services through Trinity Health Home Care Connect™ and began monitoring patients through our secure remote technology. This program still exists today. Learn more.

In 2024, we launched Beyond Balance, a fall prevention program for seniors. This comprehensive program includes tailored care plans and patient education, which empowers seniors to thrive at home. Learn more.
